What Your Practice Needs to Provide — and When
Once Duet begins managing your credentialing, there are a few key items we'll need from you to get started. This article covers what to expect, what you'll need to provide, and why acting quickly on these requests directly affects how fast your credentialing moves.
This article is for providers and practice owners going through the credentialing process with Duet — whether you're launching a new practice or adding a new provider to an existing one.
What to Expect From DuetWhen Duet kicks off your credentialing, you'll receive an onboarding email outlining everything we need from you to get started. That email will ask you to complete three things:
- 1. Provider Intake Form
A form that captures your core provider details. We ask that this is submitted within 2 business days of receiving the request.
- 2. CAQH ProView profile
Create or update your profile and share your CAQH ID with us (more on this below).
- 3. Medicare surrogate access
Grant Duet access to your Medicare account so we can manage enrollment on your behalf (more on this below).
The faster these are completed, the sooner we can submit your applications to payers — and the sooner your effective dates can be established.
What You Need to ProvideProvider Intake Form
This form collects the foundational information we need to begin credentialing on your behalf — things like your NPI, license details, practice location, and contact information. You'll receive a link to the form directly in your onboarding email.
Timeline: Please complete within 2 business days of receiving it.
CAQH ProView
CAQH ProView is a centralized database that many commercial payers use to pull provider information during the credentialing process. Instead of submitting the same documents to every payer individually, your CAQH profile acts as a single source of truth that participating payers can access directly.
If you don't have a CAQH profile yet:
- Go to the CAQH ProView registration page (proview.caqh.org/PR/Registration) and create an account
- Enter your name, NPI, license(s), and contact information
- Complete your profile with: Education, training, and work history; State license(s) and board certifications; Malpractice insurance and DEA certificate (if applicable); Practice location(s) and demographic information
- Upload supporting documents (licenses, insurance, certifications)
- Authorize payers to access your profile
- Review and attest that all information is accurate
- Once complete, you'll be assigned a CAQH ID — share this with Duet at credentialingteam@joinduet.com
If you already have a CAQH profile:
Log in, review your information for accuracy, and re-attest if needed. Then share your CAQH ID with Duet.
Important: CAQH profiles must be re-attested every 120 days. If your attestation lapses, payers cannot access your profile and your credentialing can stall. Duet will remind you when re-attestation is coming up, but keeping an eye on this yourself is a good habit.
Medicare Surrogate Access (PECOS)
For Medicare enrollment, Duet needs to be added as a surrogate on your CMS Identity & Access Management (I&A) account. This gives us the ability to submit and manage your Medicare enrollment application through PECOS on your behalf.
This is one of the most common sources of delay in the credentialing process. The sooner you complete this step, the sooner we can move forward with Medicare enrollment.
To add Duet as a surrogate:
- Log in to the CMS Identity & Access Management (I&A) System at nppes.cms.hhs.gov/IAWeb/login.do
- Follow the steps to add a surrogate to your account
- Add Kayla Brooks (kayla@joinduet.com) as your surrogate

Why Timing MattersCredentialing can't move forward until we have what we need. Every day of delay on the intake form, CAQH setup, or Medicare access is a day added to your overall timeline — and potentially a day further from your first reimbursement.

Common Questions
What if I already have a CAQH profile from a previous practice?
You can use the same profile — just log in, update your information to reflect your current practice and location, and re-attest. Make sure to share your CAQH ID with Duet.
What if I don't have a PECOS account?
New providers may need to create one before adding a surrogate. Contact us at credentialingteam@joinduet.com and we'll walk you through it.
Do I need to re-attest my CAQH profile even if nothing has changed?
Yes. CAQH requires re-attestation every 120 days regardless of whether your information has changed. If you don't re-attest, your profile becomes inaccessible to payers.
I completed the intake form — what happens next?
Duet will review your submission and begin preparing applications for your target payers. We'll reach out if anything is missing or if we need additional documentation.
